Summer Conference Registration
Registration begins May 1 and ends June 2. Late fees are assessed after June 2, and absolutely no registration will be accepted after June 10. Classes are assigned on a first come-first served basis, so get your registration in early! Registration forms must be turned in to your county Extension office.
All participants and chaperones should complete the following steps to register for Summer Conference.
|1. Read the class descriptions and decide which classes you would like to take.||Class Descriptions|
|2. Print and fill out the Class Sign-up Form. Make sure classes are offered at the times you request them. Parents/guardians must sign this form for youth. (Chaperones need to complete this form, too. We will try to match you with classes you would like to chaperone.)||Class Sign Up Form|
|3. Print and fill out the Registration Form.||Registration Form|

|5. Print, read, and sign the Code of Conduct Form.||Code of Conduct|
|6. Print, read, and sign the Climbing Wall waiver form if you have requested that class.||Climbing Wall Waiver|
|7. Write a check (made payable to the Oregon 4-H Foundation) for your portion of the registration fee. (Note: some counties may have you write the check to them.)|
|8. Turn all forms and your check in at your county Extension Office no later than June 2. Remember that registration for classes is done on a first come-first served basis, so get your forms in early.|
|Specific for Chaperones: Read the Chaperone Position Description and be sure you have completed your background check.|
|Specific for County Agents: Fill out the County Registraton Form on June 3 when registration is complete for your county|
